These forms are for enrolling, requesting classes, or for drill ground requests

Please do not take pictures of applications and email or fax them. They are very hard to decipher. Email applications as PDF's only and fax originals only. Thanks

ALL FORMS BELOW ARE FILL-ABLE -- PLEASE FILL OUT ON COMPUTER AND EMAIL OR FAX 

Combined FF1 / FF2 and Stand-alone FF2 applications will be available when the class is announced

The application / registration package for those who wish to attend the academy's FF1/FF2 program.

For use when a department requests a specific class be delivered in their station 

• Advanced Class Registration Form - updated 2-10-23 - 

The form needed to register for academy classes that are presently being advertised (except for FF1) (Some examples of advanced classes would be Pump Ops, Fire Instructor 1, CEVO, etc.).

• Drillground Request Form   Updated 2-21-23 

This form is necessary for when scheduling the academy drill ground (An example would be for when a department wants to schedule a smoke house drill or a car fires drill).

NJ State FF1 / FF2 Certification Form 

You must fill out this form after FF1 and FF2 in order to get your State Cert.  We will not allow a student into FF2 without the State Cert for FF1.

Request for Facility Use

This form is be used by Non-FD agencies wishing to use the facility for classes 

BE ADVISED:  Effective Sept. 1, 2022:  A No-show at a scheduled Drill ground Activity will result in a cost of staff charge to the No-Show Department
