The Monmouth County Clerk's Office provides identification cards to U.S. citizens who are residents of Monmouth County.
21 YEARS OF AGE AND OVER
- Certified birth certificate with raised seal (this is issued by the town in which you were born, (hospital record is not acceptable); naturalization certificate; permanent resident card
- Proof of Monmouth County residency (valid driver’s license, mail, utility bill)
- Proof of social security number (card, W2 tax form, pay stub, bank statement)
- Fee is $13.00 – cash, money order, Mastercard, Visa
- The hours for an identification card for Monmouth County residents 21 years of age or over are 8:30am – 4:00pm
UNDER 21 YEARS OF AGE
- Original birth certificate with raised seal (hospital record is not acceptable), or naturalization certificate
- Proof of Monmouth County residency (valid driver’s license, mail, utility bill)
- Proof of social security number (card, W2 tax form, pay stub, bank statement)
- All applicants under 18 years of age must be accompanied by a parent or legal guardian (with guardianship papers). Guardian and minor must share residence.
- Fee is $13.00 – cash, money order, Mastercard, Visa
- The hours for an identification card for Monmouth County residents under 21 years of age are 8:30am – 4:30pm
